    This 4 day course can be delivered in Centre or onsite by arrangement and is designed to introduce delegates to the fundamental principles associated with running an injection moulding machine, this course is a blend of theory and practical application sessions giving you a good understanding of the injection moulding process.

    This Course is Aimed at

    Any person whose job role involves investigating safety problems including incidents, near misses and non-conformities e.g. audit findings.

    Overview

    This flexible safety investigating training can be tailored to meet an organisation’s specific needs.  Participants will learn to effectively gather, collate and analyse evidence leading to understanding the story of what has happened and its root causes.  Based on this, participants will learn how to avoid poor recommendations and make effective and efficient recommendations.  The course can be extended to include basic or advanced interviewing skills, critical to effective investigation.

    Programme Content and Key Areas

    Investigation skills (1 or 2 days for up to 12 persons) covers

    • Data Collection
      • Discussion of observation
      • Interviewing
      • Checking records
      • Reviewing documentation
    • Human Factors (optional)
      • The role of human error in causing incidents and the types of error people make
    • Data Collation
      • Briefing and practical exercises using ECFA, a visual problem solving and story telling technique
    • Data Analysis
      • Practical exercise on identifying gaps in available information requiring further investigation and intervention points for effective and efficient recommendations
    • Making Recommendations
      • Practical exercises on creatively generating ideas for recommendation and evaluating their likely efficiency and cost-benefit
    • Improving In-House Investigation (optional)
      • Briefing on methods of improving in-house investigation including revising accident and incident reporting procedures, introducing nonconformity procedures and communicating learning


    Interviewing Skills (option – 1 or 2 days for up to 6 persons) covers

    • Interview Techniques
      • A series of briefings on the basic interview techniques required in all types of interview
      • A briefing on the biases that can influence interviewers and undermine interview evidence
    • Fact Finding Interviews
      • Practical exercise on the skills needed to determine facts accurately and avoid making assumptions. Course members interview each other and receive feedback
    • Nonconformity Interviews
      • Course members interview each other about nonconformities they have carried out and receive feedback
    • Accident and Near Miss Interviews
      • Course members interview each other about an accident or near miss they have had and receive feedback. Note if additional interview practice is required there can be separate accident and near miss exercises
    • Audit Interviews (optional)
      • Briefing and exercise on how to prepare interview schedules for management system audits and a practical exercise on audit interviews using the prepared schedules
    • Note that there is no role playing on this course – you cannot learn to interview properly by interviewing someone who is making it up as they go along.  All interviews are based on the participants’ and trainer’s own experiences


    For those requiring advance interviewing skills, training can be provided on the application of the structured ‘PEACE’ interview technique (Planning and Preparation, Engage and Explain, Account, Closure and Evaluation), used by the police, to health and safety investigations.  This methodology significantly increases the reliability of interview evidence.  It is strongly recommended for interviews conducted in the context of legal action.

    Investigation and interviewing skills can be independent verified following a period of practice (optional competence assurance process).
